Anxiety attacks making me forget things

I am very stressed and anxious since more than three months now. I am having great difficulty to manage with it. I have even started forgetting many things. I forget to do important things like eating on time, brushing my teeth and respecting my daily schedule at work. It is very embarassing, please help me. It's urgent.
